Circulatory Group Clinical interest: 1. In 2002, we performed 41 cases of PTCA (typeB and type C ). 2. Recently obstructive sleep apnea syndrome attracts a great deal of attention in Japan, the test of polysomnography became increasing gradually, the number of sleep study were 57 in 2002. 3. Because a significant number of the deaths caused by acute (massive) pulmonary thromboembolism occur within the hours after the onset of symptoms, one of the most important things for us is the early diagnosis and treatment for acute pulmonary thromboembolism. Research interest: Recent studies have provided the evidence that oxidative stress play a critical role in the development and progression of atherosclerosis. We are also interested in this field. At first, we focused on the regulation of glutathione, which is a major antioxidant in cells, in macrophage. Miyahara et al. reported Nε(carboxymethyl)lysine, a major advanced glycation end products, regulated the gamma-glutamylcysteine synthetase (gamma-GCS), which is a rate limiting enzyme of glutathione synthesis, mainly through the transcription factor of AP-1 (Biochem Biophys Res Commun. 2002). Muroya et al. demonstrated the regulation of gamma-GCS under the exposure of oxidized LDL (Biochem Biophys Res Commun. 2003). Oxidative stress is also involved in the pathway of angiogenesis. So, we are now doing the experiments about the mechanism of the angiogenesis, especially the phosphorylation and/or transactivation of the receptors of growth factors. As a clinical study, the survey of polymorphism of antioxidant gene is on going now. On the other hand, we have studied the pulmonary circulation of patients with pulmonary thromboembolism and chronic pulmonary diseases as a clinical research. Recently Yoshinaga et al. demonstrated the utility of ECG in patients with massive pulmonary embolism (Circ J. 2003). Sonoda et al. reported the new strategy of long-term treatment of chronic pulmonary thromboembolism patient (Intern Med. 2002). Now we are member of the Japanese Society of Pulmonary Embolism Research group. Above mentioned, we have been doing the various studies from basic as well as the clinical viewpoint. Research project encompass the following areas 1) Evaluation of venous thromboembolism in patients with nephrotic syudrome 2) Identification of the disease-susceptible and drug-responsible genes for ischemic heart disease 3) Intracellular calcium homeostasis modulates transcriptional regulation of protein phosphatase 2Ac Staff Yoshiyuki Miyahara, M.D. Tie Yasuoka, M.D. Akiyo Ninomiya, M.D. Publications in 2002 A-a 1.
